The following application forms are needed to apply for admission to Orinda Academy.

The 2012-2013 Application form, Student Application form, and personal statement should be sent directly to the Orinda Academy Admissions Office. 

The Recommendation forms should be provided to your current principal and teachers.

• (6-8) Middle School Teacher Recommendation & Transcript Request- download form

• (9-12) Transcript Request:  The signed form should be provided to your child’s current school office or registrar- download form
